Formula

RR≥22 (1) + Altered mentation (1) + SBP≤100 (1). Score ≥2 = high risk.
  • Per Sepsis-3: qSOFA ≥2 outside the ICU identifies patients likely to have a prolonged ICU stay or in-hospital death.
  • qSOFA is a screening tool; SOFA score is used for diagnosis and severity.
  • A negative qSOFA does not rule out sepsis.

Frequently asked questions

Can I use qSOFA to diagnose sepsis?
No. qSOFA is a rapid screening tool. Sepsis-3 diagnosis requires suspected infection + SOFA ≥2 (from baseline). Use qSOFA to identify patients who need urgent evaluation.
