qSOFA Score (Sepsis Screening)
Result
qSOFA Score 0
Interpretation Low risk (but clinical judgement required)
Calculate the quick SOFA (qSOFA) score as a rapid bedside sepsis screening tool.
Formula
RR≥22 (1) + Altered mentation (1) + SBP≤100 (1). Score ≥2 = high risk.
- Per Sepsis-3: qSOFA ≥2 outside the ICU identifies patients likely to have a prolonged ICU stay or in-hospital death.
- qSOFA is a screening tool; SOFA score is used for diagnosis and severity.
- A negative qSOFA does not rule out sepsis.
Altered mental status + tachypnoea
Inputs
- Respiratory Rate ≥ 22 breaths/min: 1
- Altered Mentation (GCS < 15): 1
- Systolic BP ≤ 100 mmHg: 0
Score = 2. High risk — consider sepsis evaluation.
Frequently asked questions
Can I use qSOFA to diagnose sepsis?
No. qSOFA is a rapid screening tool. Sepsis-3 diagnosis requires suspected infection + SOFA ≥2 (from baseline). Use qSOFA to identify patients who need urgent evaluation.
